Live Well at Home's Care Navigator is an inside sales position whose primary role is to meet with and evaluate client needs, sell and close home care business and respond to customer requests and concerns within their assigned areas. Additionally, this position manages the success and longevity of our clients by ensuring that we provide continuous quality care. They are responsible for building and maintaining positive relationships with their assigned clients by making customer service calls, addressing inquiries or concerns, and resolving any client complaints

 

ESSENTIAL JOB FUNCTIONS

  •  The Care Navigator  will meet with and evaluate client needs, sell and close home care business and respond to customer requests and concerns within their assigned areas.

    • Complete the client's Care Plan and all related documentation to properly document the sale and launch a new client
    • Be proactive and make monthly customer service calls to all assigned clients in order to assess client and family satisfaction
    • Make customer service calls to all assigned clients who have received a new caregiver to ensure satisfaction
    • Follow up on pending clients and clients on hold
    • Follow-up on all Prospects that have not been closed
    • Answer any customer service calls and address any inquiries or concerns
    • Use problem solving skills to resolve any client complaints, safety concerns, or caregiver issues.
    • Be proficient in the use of LWH ClearCare software
    • Other duties include being a team player by assisting with staffing, prospect calls, answering phone calls, etc.
    • Handle incoming inquiry calls, effectively selling LWH services and convert the inquiry call to an in-home assessment

 Education, Experience, Knowledge, Skills, Abilities and Availability

  • The Care Navigator shall be an individual who has a minimum of two years of experience in the home care industry.
  • Must be creative, dynamic, self-motivated, and have a pleasant and helpful disposition.
  • Position requires on-call availability, to handle after-hours calls on a rotational basis, usually every 4 to 5 weeks
  • Includes    client    assessments within the greater Sacramento area.
  • Must possess effective written and verbal communication skills, problem solving skills, and apply good judgment based on the principles of sound management.
  • Read, write, speak, and understand English as needed for the job.
  • Have a valid driver's license and use of an insured automobile or access to adequate transportation
  • Perform other related duties incidental to the work described herein.
  • Ability to multi-task and manage multiple projects and priorities in a quick and efficient   manner.
